Student Experience Form

At MassBay, we are committed to creating a positive, inclusive, and supportive learning environment for every student.  The College values your voice and welcomes your feedback as an important part of improving the student experience.

Students are encouraged to share their thoughts through the Student Experience Form.

When to Use the Student Experience Form

You can use this form to:

  • Share a positive experience at the College, including recognizing a College employee for their support or service.
  • Express concerns about any topics related to your experience at the College.
  • Suggest improvements to the overall student experience

This form is not intended for:

  • Filing a student grievance: If you wish to file a student grievance or for questions regarding retaliation, please visit the Student Grievance Procedure page, as outlined in the MassBay Student Handbook.
  • Reporting an emergency or safety issue: For urgent safety concerns, contact Campus Police at 781.239.2222 or dial 911.

If you are seeking to file a complaint regarding alleged discrimination or sexual violence (including but not limited to sexual harassment, sexual assault, dating violence, domestic violence, or stalking) please use the  . How to Submit Your Feedback

1. Complete the Online Form
Fill out the Student Experience Form with as much detail as possible.

2. Review and Processing
Your submission will be reviewed by the Dean of Students Office.  If needed, it may be referred to the appropriate office for action.

3. Follow-Up (If Applicable)
If you choose to provide your contact information, a staff member may reach out to discuss your feedback or next steps. You can also schedule a time to meet with someone from the Dean of Students Office.  Click Here.

Frequently Asked Questions (FAQs)

Submissions are not anonymous. Your name is shared with the Dean of Students Office.  However, you may request that your name not be shared with any individual or office mentioned in your submission.

Please note: In some cases, withholding your name may limit the College鈥檚 ability to respond fully or act. In the case of Title IX concerns, we are required to share your name with the Associate Dean of Equity Assurance, Lisa MacDonald. For information, please visit   

In other rare instances, the College may be legally required to provide the student鈥檚 name. 

The Dean of Students Office will review your submission and, if appropriate, may contact you for additional information. In some cases, the matter may be referred to a supervisor for appropriate follow-up.
